- Landline phone or gas bill
- Rates bill, tenancy agreement, or cable TV bill
- Proof of Income:
- Fixed deposit (HKD 350,000 for 3 months)
- Or recent 3-month bank statements/passbook showing monthly income ≥ HKD 15,000
- Or latest tax assessment showing annual salary ≥ HKD 180,000
- Family Member Details: Name, year of birth, HKID number, and relationship to the employer
- Existing/Past Helper Details: Name, HKID number, and work visa expiry date
- Housing Department Consent (for public housing residents): Copy of tenancy agreement
